Adam Smasher could be defined as a "functioning cyberpsycho". Adam has an abnormally high tolerance for cyberware, but he is still as insane as any cyberpsycho. He simply is able control himself. It could be that his mechanical body feeds his brain immunoblockers constantly or has some other mechanism in place that keeps him in check.
Adam is one of the few people who loves the feeling of being insane. He loves the feeling of superiority being a borg and is disgusted with flesh and humane concepts like mercy. He is professional when dealing with Arasaka personal, but in battle he is a ruthless psychopath.
Adam also already had psychopathic tendencies before becoming a borg and enjoyed killing people.
I think it's a combination of these factors. But Adam already is a cyberpsycho. Just not an uncontrollable unhinged chrome junkie. Instead he is a very tactical, calculating psychopath.
Theoretically David could have been saved; in the Cyberpunk 2077 universe, cloning limbs can be done during a regular doctor's visit. Though in his case, they would probably need to have first hooked him up to life support and given him a bunch of psychotherapy. (typically cyberpsycho treatment takes 6 months of therapy)
